A Case of Cancellous Bone Chips Graft on Jaw Bone


Abstract
1) A 38 years old man was primarily operated for compound ruptured fracture of mandibular corpus with frontal and left bicuspid area of the mandible due to bomb explusion.
After wound healing, bone graft operation was made secondarily.
The graft bone which was taken from right iliac bone of this patient, was wide and thin cancellous bone of several chips.
The author gained a good finding on the bone graft, and had been observed for 19 months since the operation.
The following results were obtained.
2) By this case, the author could belive that a cancellous bone graft was good method more than a cortical bone graft.
3) The fixation of this bone graft was depended upon the intermaxillary ligature only.
4) When the bone chips was put on the graft bed, between each bone chips and graft bone was tightly contacted with wide surface without large space.
5) First of all, basic bridging was made of a long cortical bone chip, and then the cancellous bone chips was put on the cortical bone chip according to the out form.
6) Any metalic material (wire, pin, screw, net or plate etc) was not inserted into the graft tissue area.
7) When the nasal trachea tube was inserted in order to do general anesthesia, mandibular left second bicuspid was fell down accidentlly.
From the result, the graft bed tissue was perforated into the oral cavity.
Nevertheless, the progress was very good with suitable antibiotic treatment after the operation. From this fact, the author belive that a possibility of bone graft intraorally was suggested.
8) The ligature wire was removed after 67 days since the bone graft.
After removing the wire, without any space maintain-appliance was fitted through 17 months, but mandibular bone did not change the previous form.
This fact suggest that the osteogenesis was accomplished already when the wire was removed.
9) Partial denture was inserted on edeutulous area after 19 months from the bone graft, on account of the economic circumstance of the patient.
